Surest: This plan covers services from almost any doctor or hospital with a discount when using a provider from the PPO network. The out of pocket expenses are higher when using a provider not in UnitedHealthcare’s PPO network. Referrals are not usually required to see specialty physicians.

Find a Provider:

  1. Go to the UHC provider website:
  2. Click on “find a network”,
  3. Click on “Medical Directory”
  4. Click on “Employer and Individual plans”
  5. Scroll and select the "Choice Plus"network
  6. Ensure the location is set to the location of the provider in which you are searching
  7. Search for specific providers by typing name into search bar
  8. Look for a provider by category by clicking one of the categories

UnitedHealthcare has the largest proprietary provider network with over 1.2 million providers, over 5,400 hospitals, and 2,300 convenient care clinics.

Prescription coverage for this plan is through CVS Caremark and Prescription information will not be included on your Surest Health Insurance ID. Members will get a card directly from CVS Caremark.

Once you are enrolled you will receive an ID Card from Surest and CVS within 10-14 business days. If you need a card sooner you can register for the Surest Account and see your card there.

On this plan you can enroll in Medicare Part A, but decline Medicare Part B.

This plan is considered a credible plan by Medicare, so when you or your dependent are eligible for Medicare part B, you can decline the coverage and then when your coverage with the University or any credible plan ends a Special enrollment period will open to enroll in Medicare Part B.

You will need a form from the plan sponsor, for example if you are on this plan and retire are 66, 91 will complete the form for you.
